Licensed Practical Nurse LPN

Anchorage Rehabilitation and Wellness Center is seeking Licensed Practical Nurses to join our post-acute rehabilitation team. In this role, you will provide direct nursing care to residents, promote their health and independence, and collaborate with physicians and multidisciplinary team members to deliver compassionate, high-quality care.

Responsibilities

• Establish personal rapport with residents and their families to identify and understand care requirements
• Monitor and chart resident health information, administer medications, and oversee assigned staff
• Provide emotional, psychological, and spiritual support to residents, families, and friends
• Establish patient care goals and teach residents and families about their conditions, medications, and self-care skills
• Measure health outcomes against care goals and standards, making or recommending necessary adjustments
• Resolve resident problems and needs using multidisciplinary team strategies
• Maintain safe and clean working environments by complying with procedures, rules, and regulations
• Protect residents and employees by adhering to infection-control policies, medication administration procedures, and controlled substance regulations
• Document resident care services in resident and department records
• Maintain resident confidentiality and protect facility operations
• Maintain professional and technical knowledge through educational workshops, professional publications, and participation in professional societies
• Communicate effectively with healthcare teams, respond to requests, and participate in team problem-solving
• Contribute to team efforts by accomplishing related results as needed

Required Qualifications

• Graduate of an approved college or school nursing program
• Current state license as a Licensed Practical Nurse

Preferred Qualifications

• At least one year of experience in a long-term care environment
