Stellar Care and Services, LLC is seeking a Licensed Practical Nurse (LPN) for a full-time administrative nursing position supporting adults with intellectual and developmental disabilities receiving services through the HCBS-DD Medicaid waiver.
This is not a bedside nursing role. Instead, you will provide administrative and clinical support including chart auditing, developing medical protocols, conducting qualified medication administration (QMAP) training, performing self-medication assessments, and delivering other specialized medical training. You will work closely with the interdisciplinary team, program professionals, individuals served, families, guardians, and community stakeholders to ensure health needs are met in accordance with acceptable medical practices and applicable Medicaid and state regulations.
Schedule
Full-time, four days per week, Monday through Thursday, 8:00 AM to 5:00 PM (32 hours per week).
Reporting
You will report to the Agency Director.
Responsibilities
- Work with the person served team and physicians to develop and manage care plans
- Instruct patients and families in proper treatment as necessary
- Help patients take steps to improve or maintain health
- Respond to inquiries in a responsive and professional manner
- Perform and conduct assessments of individuals served ability to self-medicate
- Provide nurse delegations for finger pricks and injectables when members are not able to self-administer
- Develop individualized protocols pertaining to seizures, feeding, breathing, choking, G/J-tube, and other medical-related support needs
- Provide qualified medication administration training (QMAP) to all employees and contractors, developing a class in each office monthly and ensuring certificates are submitted to HR timely
- Monitor and audit medical charts quarterly and report discrepancies to program managers and director as needed
- Assist program director and coordinator with audit corrections
- Conduct G-tube training and semi-annual G/J-tube observations with providers, ensuring documentation is submitted timely
- Report all critical and non-critical incidents to direct supervisor immediately and write incident reports within 24 hours
- Perform all other duties as assigned
Required Qualifications
- Active license to work as an LPN or above in the State of Colorado issued by the Department of Regulatory Agencies (DORA)
- Current DORA certification in good standing, updated annually as required by Colorado law
- Ability to register with the State to obtain required credentials necessary to teach employees and contractors medication administration certification (QMAP)
- Prior experience and expertise in nursing
- Ability to use basic computer-related programs including Microsoft Office applications
- Knowledge of rules and regulations associated with medical supports of individuals with intellectual and developmental disabilities under the HCBS-DD Medicaid Waiver
- Ability to speak and write English legibly and read English appropriately when needed
- Ability to lift and transfer up to 50 pounds
- Good physical and mental health
- Physical ability to stand, walk, use hands and fingers, reach, stoop, kneel, crouch, talk, hear, and see
- Mental fortitude and stability to handle stress
- Physical and mental ability to drive a vehicle
- Ability to engage in skilled interaction with people who require behavior development to replace aggressive behavior

- Prior experience working with individuals who have a variety of intellectual and developmental disabilities
Driving Requirements
This position is designated as an unlimited driving position, though driving will be minimal. You must have a current, valid U.S. driver's license with acceptable status as defined by the employer's insurance carrier. If hired, you must comply with Stellar Care's policies and procedures regarding driving. If the position requires you to drive your own vehicle, you are responsible for your own automobile insurance. Mileage will be reimbursed.
