Licensed Practical Nurse - (LPN)

Addison Pointe Health and Rehabilitation is seeking Licensed Practical Nurses to provide daily care, health needs, safety, and comfort to residents in our long-term care facility. You will perform professional assessments, maintain documentation of resident health status, and assist with management of assigned nursing units.

Schedule

• Full-time and part-time night shifts available

Responsibilities

• Supervise, perform, and provide written and verbal care assignments, routine skills, and procedures to ensure quality resident care
• Participate in the development and implementation of resident care plans, maintain up-to-date nursing documentation, and make recommendations for care plan revisions according to facility policy
• Make daily and periodic visits to observe for symptoms and reactions to medications or treatments
• Obtain and document vital signs, intake and output, weights, diet supplements and replacements, cultures, and other clinical data
• Assist with orientation and ongoing training of staff to maintain required skill levels

Required Qualifications

• Graduate of an accredited school of nursing
• Valid LPN license in good standing according to State and Federal requirements
• Thorough understanding of the principles of safe and effective nursing practices within the State of Indiana
• Basic computer skills including knowledge of Excel and Word
• Personal integrity and ability to deal tactfully with residents, family members, visitors, government agencies, and personnel
• Ability to maintain confidentiality and protect the integrity of confidential information in compliance with Corporate Compliance and HIPAA requirements
• Knowledge of personal computers, Word, Excel, and office machines such as fax and copying machines
