Licensed Practical Nurse ( LPN ) 7PM - 7AM

Legacy Health and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents in our long-term care and rehabilitation facility. You will work the night shift (7PM - 7AM) and play a key role in supporting residents' recovery and well-being through medication administration, wound care, monitoring, and collaboration with the interdisciplinary healthcare team.

Clinical Setting

This is a full-time position in a long-term care and rehabilitation facility where you will work alongside RNs, physicians, and other healthcare professionals to deliver compassionate, resident-focused care.

Responsibilities

• Administer medications and treatments as prescribed by physicians
• Monitor and document residents' health status, including vital signs, and report changes to the supervising RN or physician
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed
• Collaborate with the rehabilitation team to support residents' recovery goals, including physical, occupational, and speech therapy plans
• Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations
• Provide emotional support and education to residents and their families
• Ensure a safe and clean environment, adhering to infection control protocols

Required Qualifications

• Current and valid LPN license
• BLS/CPR certification
• Strong clinical skills, including medication administration and wound care
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-oriented environment
• Patient-focused approach to care

Schedule

• Shift: 7PM - 7AM (12-hour shifts)
• Full-time employment
• Every other weekend off (Friday, Saturday, and Sunday)

Compensation and Benefits

• Pay: $25.00–$32.00 per hour, based on experience
• Sign-on bonus: $10,000
