Legacy Health and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents in our long-term care and rehabilitation facility. You will administer medications and treatments, monitor residents' health status, assist with activities of daily living, and collaborate with the rehabilitation team to support residents' recovery goals.
Clinical Setting
This is a full-time position in a long-term care and rehabilitation facility where you will work with an interdisciplinary team including RNs, physicians, and rehabilitation specialists.
Responsibilities
- Administer medications and treatments as prescribed by physicians
- Monitor and document residents' health status, including vital signs, and report changes to the supervising RN or physician
- Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed
- Collaborate with the rehabilitation team to support residents' recovery goals, including physical, occupational, and speech therapy plans
- Maintain accurate and timely documentation in accordance with facility policies and state and federal regulations
- Provide emotional support and education to residents and their families
- Ensure a safe and clean environment, adhering to infection control protocols
Schedule
- Shift: 7 AM to 7 PM
- 12-hour shifts
- Every other weekend off (Friday, Saturday, and Sunday)
Required Qualifications
- Current and valid LPN license
- BLS/CPR certification
- Strong clinical skills, including medication administration and wound care
- Excellent communication and interpersonal skills
- Ability to work effectively in a team-oriented environment
Compensation and Benefits
- Pay: $25.00 to $32.00 per hour, based on experience
- Sign-on bonus: $10,000
