Licensed Practical Nurse ( LPN ) 3PM - 11PM

Springdale Health and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents in our long-term care and rehabilitation setting. You will work collaboratively with RNs, physicians, and interdisciplinary team members to support residents' recovery goals and ensure the highest standards of care.

Clinical Setting

This is a long-term care and rehabilitation facility where you will care for residents requiring ongoing medical support, rehabilitation services, and assistance with activities of daily living.

Responsibilities

• Administer medications and treatments as prescribed by physicians
• Monitor and document residents' health status, including vital signs, and report changes to the supervising RN or physician
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed
• Collaborate with the rehabilitation team to support residents' recovery goals, including physical, occupational, and speech therapy plans
• Maintain accurate and timely documentation in accordance with facility policies and state and federal regulations
• Provide emotional support and education to residents and their families
• Ensure a safe and clean environment, adhering to infection control protocols

Required Qualifications

• Current and valid LPN license
• BLS/CPR certification
• Strong clinical skills, including medication administration and wound care
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-oriented environment
• Patient-focused approach to care

Schedule

• Evening Shift: 3 PM - 11 PM
• Full-Time
• Evening Shift Differential

Compensation and Benefits

• Hourly Pay Range: $27.00–$32.00 per hour (based on experience)
