Valparaiso Care & Rehab is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a long-term care and rehabilitation setting. In this role, you will develop meaningful relationships with residents and staff while utilizing your nursing skills to promote physical and psychosocial well-being. You will work as part of a collaborative care team to deliver comprehensive, resident-centered care.

Clinical Setting

This position is based at Valparaiso Care & Rehab, a senior living community serving geriatric residents requiring skilled nursing care and rehabilitation services.

Responsibilities

  • Provide direct nursing care to promote the physical and psychosocial well-being of residents
  • Manage medications with proficiency and accuracy
  • Conduct thorough assessments and accurately document changes in resident condition
  • Collaborate with nursing and other facility teams to ensure coordinated and comprehensive care
  • Create a comforting and engaging atmosphere for residents
  • Promote teamwork within the care team to meet and exceed resident needs
  • Support a respectful and positive work environment through effective communication
  • Demonstrate the facility's C.A.R.E. values (Compassion, Accountability, Relationships, and Excellence) in all interactions with residents, families, and staff

Schedule and Shifts

  • Full-time and part-time positions available
  • Evening and night shifts
  • Weekend shifts available at $40/hour

Required Qualifications

  • Current and valid Licensed Practical Nurse license in the state of Indiana
  • Proficient medication management skills
  • Ability to conduct thorough assessments and accurately document changes in resident condition
  • Excellent communication and interpersonal skills
  • Strong passion for geriatric nursing and commitment to senior care excellence

Compensation

  • Base pay: $34-37/hour
  • Weekend differential: $40/hour
