Licensed Practical Nurse (LPN) 33444

The Lakes Home Care is hiring a Licensed Practical Nurse (LPN) to provide skilled nursing care to pediatric and/or adult patients in their own homes. You will work independently in home settings, utilizing the nursing process to assess, plan, implement, and evaluate patient care according to prescribed medical orders.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Provide skilled nursing care and services in accordance with prescribed orders on each assignment
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
• Collect, analyze, and interpret data and information from health care members
• Document actual and/or potential nursing diagnoses

Required Qualifications

• Current LPN/LVN license in the state where you will practice
• Current TB test or Chest X-Ray
• Current BLS certification card

Preferred Qualifications

• One year of prior professional nursing experience
