This position is a Licensed Practical Nurse (LPN) role working the 1st shift on a 12-hour schedule in a community-based residential setting. You will provide direct resident care with a focus on medication management, care planning, and interdisciplinary communication.
Responsibilities
- Oversee medication distribution according to physician orders, ensuring strict adherence to Community medication management documentation and procedures while prioritizing resident safety
- Conduct thorough assessments and create personalized service plans tailored to individual care needs while supporting resident independence and dignity
- Foster communication between physicians, healthcare providers, and family members regarding the physical, mental, and social well-being of residents
- Contribute to quality assurance and performance improvement initiatives
- Participate in continuous learning through attendance at educational seminars, staff meetings, and other professional development opportunities
Required Qualifications
- Current state licensure as Licensed Practical Nurse (LPN)
- Current CPR certification
- Exceptional communication and interpersonal skills
- Demonstrated ability to provide care with compassion and dignity
