Licensed Practical Nurse ( LPN ) 11p - 7a Shift

Civic Center Health and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents in our long-term care and rehabilitation setting. You will work the night shift (11pm - 7am) administering medications, monitoring resident health status, assisting with activities of daily living, and collaborating with the interdisciplinary team to support residents' recovery and rehabilitation goals.

Clinical Setting

This is a full-time position in a long-term care and rehabilitation facility where you will work alongside RNs, physicians, and rehabilitation specialists to deliver compassionate, patient-focused care.

Responsibilities

• Administer medications and treatments as prescribed by physicians
• Monitor and document residents' health status, including vital signs, and report changes to the supervising RN or physician
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed
• Collaborate with the rehabilitation team to support residents' recovery goals, including physical, occupational, and speech therapy plans
• Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations
• Provide emotional support and education to residents and their families
• Ensure a safe and clean environment, adhering to infection control protocols

Required Qualifications

• Current and valid LPN license
• BLS/CPR certification
• Strong clinical skills, including medication administration and wound care
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-oriented environment

Schedule

• Full-time, night shift (11pm - 7am)
• Shift differential pay available

Compensation and Benefits

• Sign-on bonus: $10,000
• Hourly pay range: $25.00–$32.00/hour (based on experience)
