LICENSED PRACTICAL NURSE - LONG TERM CARE 11-6 - FULL-TIME EVENINGS 22021

New Bridge Medical Center is seeking a Licensed Practical Nurse for our long-term care facility. This full-time evening shift position (11 PM–6 AM) offers the opportunity to provide direct nursing care to our diverse patient population in a supportive and collaborative healthcare environment.

Clinical Setting

You will work in a long-term care setting, providing care to residents across all patient care areas. You will interact regularly with patients, families, visitors, and interdisciplinary healthcare staff.

Responsibilities

• Collect patient data from patients, families, and significant others; record information on the Initial Data Base form
• Conduct 24-hour nursing assessments and provide data to the RN for MDS inclusion
• Contribute to the formulation and updating of the plan of care and implement the plan under RN direction
• Provide direct nursing care to assigned patients, including medication administration, treatments, dressing changes, resident care, and vital signs according to established standards
• Recognize changes in patient condition and respond appropriately, notifying the charge nurse as needed
• Document pertinent information regarding patient responses and nursing interventions in the medical record
• Administer medications according to hospital policies; monitor age-specific data including medications, lab results, and diagnostic findings
• Apply principles of infection control and safety in all patient care activities
• Communicate effectively with the charge nurse and healthcare team regarding patient needs and condition changes
• Respond appropriately in crisis situations and prioritize effectively
• Report significant unit problems to the charge nurse
• Demonstrate understanding of and compliance with all regulatory agencies; assume charge nurse responsibilities in the absence of a charge nurse
• Deliver patient care with knowledge of patient growth and development appropriate to patient ages; comply with the Bill of Rights
• Participate in patient teaching and nursing research activities (CQI)
• Maintain established departmental policies, procedures, quality assurance standards, safety, environmental, and infection control standards; assess gaps and create necessary policies as needed
• Orient patients and families to the new environment; develop rapport with patients, families, and significant others
• Orient LPNs and Certified Nurse Aides as required
• Effectively utilize the hospital computer system in accordance with hospital policy
• Participate on unit committees and in CQI activities as assigned
• Participate in patient care conferences and staff development activities as required
• Assist other personnel as needed
• Perform other related duties as required

Required Qualifications

• Graduation from an accredited school of practical nursing or equivalent education as described by state education law
• Current New Jersey Licensed Practical Nurse license
• Current American Heart Association Basic Life Support (BLS) certification
• IV Certification
• Clinical nursing skills appropriate for an LPN
• Good oral and written communication skills
• Good interpersonal skills
• Ability to speak, read, and write English to the extent required by the position
• Successful completion of Progressive Care Orientation Program required

Preferred Qualifications

• Experience as a licensed practical nurse in a long-term care setting
• Experience with medication pass
• Experience with Encore system
• Experience with dressing changes

Physical Demands

• Frequent prolonged standing and walking
• Lifting, positioning, pushing, and transferring patients
• Use of protective clothing including face shield, gloves, impermeable gown, and particulate respirator

Schedule and Compensation

This is a full-time position working evenings (11 PM–6 AM). Salary is commensurate with experience within the posted range of $33.22–$39.42 per hour, plus differentials: evening shift $1.60 per hour, night shift $1.70 per hour, and weekend $1.00 per hour.
