Lacey Post Acute & Rehabilitation, a top-rated long-term care and skilled nursing facility in Lacey, Washington, is seeking Licensed Practical Nurses to join our clinical team. In this role, you will provide compassionate nursing care to seniors, working collaboratively with RNs, CNAs, and interdisciplinary teams to support residents' physical, emotional, and psychological well-being.

Clinical Setting

This is a long-term care and skilled nursing facility serving seniors requiring post-acute and rehabilitation services.

Responsibilities

  • Provide nursing care to residents in accordance with standards of practice, physician orders, center policies and procedures, and state, federal, and local guidelines
  • Administer medications and treatments with precision and compassion
  • Collaborate with RNs, CNAs, and interdisciplinary teams
  • Support residents' physical, emotional, and psychological well-being
  • Assist with resident comfort
  • Collect samples and monitor catheters
  • Provide wound care
  • Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration
  • Keep resident patient information confidential and adhere to all policies, procedures, federal, state, and local requirements, and professional standards
  • Help coach staff as necessary

Schedule and Shifts

  • Full-time, part-time, and per diem positions available
  • 8 and 12 hour shifts

Required Qualifications

  • Current LPN license in the state of employment
  • CPR/BLS certification (or willingness to obtain)
  • Positive attitude, reliability, and strong clinical judgment

Preferred Qualifications

  • Previous long-term care or skilled nursing facility experience
