Licensed Practical Nurse in Suwanee, GA

AccordCare is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in the home health setting in the Suwanee, GA area and surrounding communities. This role offers the opportunity to deliver one-on-one clinical support that promotes client health, independence, and dignity while working independently in clients' homes.

Responsibilities

• Provide skilled nursing care tailored to each client's unique needs
• Monitor health conditions and document changes in clinical status
• Administer medications and perform wound care
• Deliver G-tube, trach, or vent support when required
• Educate and guide clients and families to support ongoing health at home
• Communicate effectively with the care team to ensure continuity of care

Required Qualifications

• Active LPN license in good standing with the Georgia Board of Nursing
• CPR certification
• At least one year of experience in a clinical setting
• Skilled in EMR documentation with strong organizational ability
• Ability to work independently in a home environment
• Valid driver's license with reliable transportation
• Experience with G-tube, trach, or vent care

Schedule

• Variable start days with most shifts beginning at 7:00 am daily
• Flexible scheduling available
