Licensed Practical Nurse - Hospital at Home

BAYADA Home Health Care is seeking Licensed Practical Nurses for our HICU® Health Services Hospital at Home program in the New Castle, DE area. This role offers the opportunity to care for infants and young children transitioning from hospital to home settings.

As a Hospital at Home Nurse, you will work in a team-based environment as an extension of the hospital care team, delivering skilled nursing care to pediatric clients in their homes. You will provide quality care during the initial weeks of homecare, deliver extensive training to families and ongoing care teams, and support children in living at home with appropriate medical support.

Clinical Setting

This position serves infants and young children requiring high-acuity care in home settings, with a focus on pediatric populations previously cared for in ICU or high-tech hospital environments.

Responsibilities

• Deliver one-on-one skilled nursing care to pediatric clients in home settings
• Provide extensive family training and education
• Support ongoing care team coordination
• Manage infusion therapy and tracheostomy/ventilator care
• Act as an extension of the hospital care team during transition to home

Required Qualifications

• Current Licensed Practical Nurse (LPN) license
• Minimum one year of verifiable work experience as an LPN
• Experience in NICU, PICU, ICU, or home care trach/vent settings
• Demonstrated competence in infusion therapy and tracheostomy/ventilator care (demonstrated in home or lab setting)

Schedule and Employment

• Full-time and part-time scheduling options available
• Weekly pay

Compensation and Bonuses

• Salary range: $45-$50 per hour
• $3,000 sign-on bonus
• $2,400 nurse referral bonus
