Pinnacle Home Care, Florida's largest independent Medicare-certified home health provider, is hiring Licensed Practical Nurses (LPN) for weekend PRN shifts serving patients in The Villages and Oxford areas. In this role, you will deliver skilled nursing care in patients' homes under RN supervision, performing curative, rehabilitative, and preventive care tasks as outlined in individualized care plans.

Responsibilities

• Administer skilled nursing care under RN supervision
• Perform curative, rehabilitative, or preventive care tasks according to patient care plans
• Utilize clinical expertise and nursing education to make informed, independent decisions that support patient care and positive outcomes
• Complete visit reports promptly and accurately in the electronic medical record system to ensure regulatory compliance and high-quality documentation
• Confirm daily visit schedules to coordinate seamlessly with other staff
• Demonstrate awareness of and participate in the agency's Quality Assurance and Performance Improvement (QAPI) program as required
• Understand and adhere to the agency's process for managing and addressing customer complaints
• Maintain acute awareness of patient rights and ensure these rights are upheld in accordance with agency service agreements and admission policies
• Understand agency policies regarding appropriate patient discharge and reasons for discharge

Required Qualifications

• Current LPN or LVN license (state-specific)
• Graduate of an accredited nursing program
• Valid BLS certification
• Minimum one year of nursing experience
• Valid driver's license and access to a reliable, insured vehicle
• Ability to visit homes that may not be wheelchair accessible
• Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques
• Ability to engage in physical activities including walking, standing, stooping, and occasional lifting or turning of patients
• Excellent communication, organizational, and time management skills

Preferred Qualifications

• Home health or medical/surgical nursing experience
