VitalCaring is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide direct patient care in home health settings within the Port Charlotte branch service area. In this field-based role, you will deliver skilled nursing services to patients in their homes, working under the supervision of a Registered Nurse while collaborating with an interdisciplinary care team.

You will be responsible for implementing individualized care plans, delivering treatments, monitoring patient conditions, and educating patients and caregivers on self-care and disease management. This position offers the opportunity to build meaningful patient relationships while maintaining independence in managing your daily schedule and workload.

Responsibilities

  • Provide direct patient care in accordance with state Nurse Practice Act requirements
  • Implement and support care plans developed by the RN
  • Deliver skilled nursing services including treatments, medication support, and wound care
  • Monitor patient condition and report changes to the RN and care team
  • Educate patients and caregivers on self-care, treatments, and disease management
  • Maintain infection control and proper use of equipment and supplies
  • Document patient care accurately and timely within the electronic medical record (EMR)
  • Communicate effectively with physicians, nurses, and interdisciplinary team members
  • Participate in case conferences, team meetings, and ongoing training
  • Support coordinated care through consistent communication across the care team

Required Qualifications

  • Active LPN/LVN license in the state of practice
  • Graduate of an accredited practical/vocational nursing program
  • Current CPR certification
  • Valid driver's license and reliable transportation
  • Basic proficiency with EMR systems
  • Strong clinical and observational skills
  • Clear communication and patient education abilities
  • Ability to follow care plans while recognizing and reporting changes in condition

Preferred Qualifications

  • Minimum one year of nursing experience (Home Health, Med-Surg, or Acute Care preferred)
  • Experience in home health or community-based care
  • Strong patient engagement and relationship-building skills
  • Ability to work independently while staying connected to a team
  • Familiarity with Homecare Homebase (HCHB)

  • Field-based role providing care in patients' homes within an assigned territory
