Licensed Practical Nurse-Home Health- Oshkosh

Advocate Health is seeking a Licensed Practical Nurse for home health services in Oshkosh, Wisconsin. In this full-time role, you will provide direct patient care in patients' homes and assisted living facilities under the supervision of a registered nurse. You will work Monday through Friday, 8:00 AM to 4:30 PM, with no weekend or call requirements and holiday rotation.

Responsibilities

• Assure and improve quality of care by facilitating patients' and families' rights to receive quality, cost-effective care following evidence-based standards
• Utilize appropriate resources to address situations that may negatively impact patient and family outcomes
• Participate in efforts to reduce risk and improve patient safety
• Document thoroughly and decisively to support reimbursement and regulatory requirements
• Recognize changes in clinical situations and consult with the RN regarding appropriate actions
• Make sound clinical decisions and provide care for patients with complex problems
• Prioritize and organize patient care based on the established plan of care
• Ensure clear hand-off communication using techniques such as SBAR and 5P's to achieve optimal health outcomes
• Actively participate in interdisciplinary case conferences as appropriate
• Develop and maintain therapeutic relationships with patients and families throughout the healthcare continuum
• Demonstrate adaptive communication styles based on individual patient needs
• Promote patient and family knowledge and confidence in managing self-care, disease processes, medications, procedures, and infection control prevention
• Participate in implementing individualized approaches to the plan of care, including assessment of physical, psychosocial, comfort, and wellness needs under RN supervision
• Assess patient and family readiness and involve them in goal setting, planning, and implementation of care
• Provide care that exceeds patient and family expectations
• Collaborate with the RN to implement plans that maximize efficiencies and conserve resources
• Assess patient and family readiness to learn and modify educational approaches as needed
• Provide and adapt appropriate resource materials to achieve educational outcomes

Clinical Setting

You will work in patients' homes and assisted living facilities within the Oshkosh geographical area.

Required Qualifications

• Current LPN license in Wisconsin
• CPR certification
• Ability to demonstrate compliance with state continuing education requirements
• Typically requires one year of experience performing assessments and providing treatments
• Basic knowledge of computer use including Microsoft Word and email
• Ability to learn and use computer-based scheduling and documentation systems
• Professional verbal and written communication skills with strong documentation abilities
• Good interpersonal skills
• Good time management and self-organization skills
• Proficiency in obtaining lab draws and reporting results to the primary nurse or clinical manager

Physical Requirements and Working Conditions

• Extensive travel within geographical boundaries in all weather conditions and on all types of roads, including expressways
• Must provide or arrange own transportation to and from patients' homes and branch offices
• Ability to work rotating weekends and holidays
• Must adhere to scheduled days and hours of work based on patient care commitments and assigned workload
• Must attend mandatory informational, educational, and branch meetings
• Full range of body motion required, including bending, sitting, standing, climbing stairs while carrying objects, pulling, pushing, reaching, carrying, and lifting
• Ability to assist and maintain patient safety with ambulation on flat surfaces and stairs
• Ability to transfer patients using standard pivot transfers or with transfer board or Hoyer lift
• Ability to provide treatment on standard beds in the home environment
• Ability to lift up to 35 pounds without assistance; for patient lifts over 35 pounds or when patients are unable to assist, patient handling equipment must be used with at least one other associate when available

Schedule

• Full-time position
• Monday through Friday, 8:00 AM to 4:30 PM
• No weekend or call requirements
• Holiday rotation

Compensation and Benefits

• Pay range: $25.30 to $37.95 per hour
• Base compensation determined by qualifications, skills, relevant experience, and training
