Assured Home Health, part of LHC Group and the Optum family of businesses, is seeking a Licensed Practical Nurse to provide quality patient care and skilled nursing services in patients' homes throughout Washington state.
As a Licensed Practical Nurse in Home Health, you will deliver clinical services within the scope of practice as defined by Washington state nursing laws. You will work collaboratively with the healthcare team to implement individualized plans of care while maintaining all applicable infection control procedures.
Compensation structure includes two types of pay depending on work performed. When conducting patient visits, you will be paid per visit point rate compensation calculated by multiplying your per visit point rate by the productivity points accrued for various visit types. Each visit type is assigned a certain number of productivity points inclusive of direct and indirect patient care activities. You will be paid your hourly rate for certain non-visit activities such as orientation. Annual total cash compensation assumes full-time employment at 40 weekly hours at full productivity and is based on factors including local labor markets, education, work experience, and may increase over time based on productivity and performance. The number of visits performed each week will vary based on individual productivity targets and assigned productivity points.
