The opportunity

Job Description

Assured Home Health, part of LHC Group and the Optum family of businesses, is seeking a Licensed Practical Nurse to provide quality patient care and skilled nursing services in patients' homes throughout Washington state.

As a Licensed Practical Nurse in Home Health, you will deliver clinical services within the scope of practice as defined by Washington state nursing laws. You will work collaboratively with the healthcare team to implement individualized plans of care while maintaining all applicable infection control procedures.

Responsibilities

  • Provide high-quality clinical services within the scope of LPN practice as defined by state law, in accordance with the patient's plan of care
  • Perform technical skilled care in patients' homes
  • Document patient visits thoroughly and completely in the electronic medical record per regulatory and payer requirements, utilizing the agency's electronic visit verification program as applicable
  • Participate actively in the agency's care coordination process, including timely reporting of patient needs and changes in condition
  • Attend patient case conferences and communicate effectively with patients, families, physicians, other clinicians, agency management, case coordinators, and community case managers as applicable
  • Function professionally in any home situation regardless of patient age, race, creed, color, sex, disability, or financial condition

Required Qualifications

  • Current and unrestricted LPN licensure in the state of Washington
  • Current driver's license and vehicle insurance
  • Access to a dependable vehicle or public transportation

Preferred Qualifications

  • Current CPR certification or ability to complete within 90 days of hire

Compensation

  • Annual total cash target pay: $51,738 - $77,606
  • Hourly rate: $24.87 - $37.31
  • Per visit point rate: $29.85 - $44.77

Compensation structure includes two types of pay depending on work performed. When conducting patient visits, you will be paid per visit point rate compensation calculated by multiplying your per visit point rate by the productivity points accrued for various visit types. Each visit type is assigned a certain number of productivity points inclusive of direct and indirect patient care activities. You will be paid your hourly rate for certain non-visit activities such as orientation. Annual total cash compensation assumes full-time employment at 40 weekly hours at full productivity and is based on factors including local labor markets, education, work experience, and may increase over time based on productivity and performance. The number of visits performed each week will vary based on individual productivity targets and assigned productivity points.
