Licensed Practical Nurse Home Health

Intermountain Health is seeking a Licensed Practical Nurse (LPN) to join our Home Health team in the Las Vegas area. In this role, you will provide direct patient care in patients' homes under the direction of a Registered Nurse and in collaboration with physicians and other healthcare professionals. You will deliver skilled nursing services, patient education, medication management, and ongoing monitoring to help patients achieve the highest level of independence and comfort in their own homes.

Clinical Setting

You will work in home health, providing care in patients' homes throughout Las Vegas and surrounding communities. This role offers the opportunity to build meaningful connections with patients and families while delivering compassionate, skilled care in a familiar and supportive environment.

Responsibilities

• Provide skilled nursing care in patients' homes
• Monitor patient conditions and report changes to the RN case manager or physician
• Assist with medication management and patient education
• Reinforce treatment plans and teach patients and caregivers
• Perform wound care and other treatments within LPN scope of practice
• Document visits electronically and coordinate care with the interdisciplinary team
• Travel between patient homes throughout the service area

Schedule

• Monday through Friday day shift
• 40 hours per week
• Mileage reimbursement provided

Required Qualifications

• Current LPN license in the state of practice (Nevada)
• Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire or obtained within 60 days of hire with prior approval
• Advanced Care Life Support (ACLS) certification if applicable to unit
• Approved IV certification as required by facility
• Clean driving record as required by facility
• Ability to connect to the company's online database twice daily as required by facility

Preferred Qualifications

• One year of nursing experience
• Previous home health, hospice, long-term care, or community-based nursing experience
• Experience providing patient and caregiver education

Physical Requirements

• Ability to see and read information, labels, and assess patient needs; operate monitors; identify equipment and supplies
• Frequent verbal communication with patients, caregivers, and healthcare providers; ability to hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergencies
• Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision; frequent computer use and typing for documentation
• Ability to lift and utilize full range of movement to transfer patients; bend to retrieve, lift, and carry supplies and equipment of varying weights, including heavy items
• Ability to walk and assist with transporting and ambulating patients; obtain and distribute supplies and equipment; push and pull gurneys and portable equipment, including heavy items
• Ability to navigate crowded and busy rooms with equipment and power cords
• May be required to stand in a stationary position for extended periods
• Expected to drive a vehicle, which requires sitting, seeing and reading signs, traffic signals, and other vehicles

Compensation

• Hourly range: $26.74 - $41.28 (actual rate dependent upon experience)
