Brooks Rehabilitation and Home Health Division is seeking a Licensed Practical Nurse to provide nursing care to patients in the home health setting. You will work under the direction of a registered nurse, follow established plans of care, and participate in care planning while adhering to company policy and state law.
Responsibilities
- Document observations and care provided according to company policies and procedures
- Administer medications
- Provide basic nursing care to rehabilitation patients and assist with care for acutely ill patients, considering patient age, developmental needs, and understanding
- Admit, transfer, and discharge patients
- Observe, record, and report subjective patient symptoms
- Assist the RN in providing physical and emotional support to patients
- Manage dietary and fluid needs including nourishment, feeding, tube feeding, and monitoring IV fluids
- Maintain accurate fluid intake and output records
- Take and record vital signs including temperature, pulse, respirations, and blood pressure
- Adhere to patient rights policies and procedures
- Maintain a clean and safe environment and identify and report hazards
- Meet all mandatory in-service requirements
- Follow universal precautions when delivering patient care
- Participate in on-call rotation
