Job Description
Grafton Oaks Rehabilitation and Nursing Center is hiring Licensed Practical Nurses to deliver care to residents in a long-term care and skilled nursing setting. Under new management, the facility is focused on building a supportive team environment while maintaining excellent resident care standards.
Clinical Setting
Grafton Oaks is a rehabilitation and nursing center located at 405 Grafton Ave, Dayton, OH 45406. The facility serves long-term care and skilled nursing residents.
Responsibilities
- Deliver safe, effective nursing care in accordance with resident care plans
- Administer medications and treatments per physician orders
- Monitor residents for changes in condition and report as needed
- Maintain accurate documentation and ensure compliance
- Build relationships with residents and families
Required Qualifications
- Active LPN license in Ohio
- Great communication and reliability
- Team player mentality
Preferred Qualifications
- Long-term care or skilled nursing experience
