Job Description

Touchmark on West Prospect, Appleton's leading full-service senior living community, is seeking a Licensed Practical Nurse (LPN) to join the Health Services team. In this full-time role, you will provide direct, person-centered nursing care to residents in Assisted Living and Memory Care settings, supporting their physical, emotional, and overall well-being while working collaboratively with RNs and clinical leadership.

Clinical Setting

You will work in a senior living community providing care to older adults in Assisted Living and Memory Care environments. The role emphasizes building lasting relationships with residents and families while maintaining individualized care plans.

Responsibilities

  • Provide direct nursing care within the LPN scope of practice
  • Administer medications and treatments
  • Observe, assess, and document resident conditions and care provided
  • Communicate effectively with residents, families, and the healthcare team
  • Support individualized care plans for Assisted Living and Memory Care residents
  • Assist with training and supporting medication aides and new team members
  • Model organizational values and uphold Gold Standards of care

Required Qualifications

  • LPN degree from an accredited nursing program
  • Current state LPN licensure
  • CPR/First Aid certification or ability to maintain per state requirements
  • Strong clinical, communication, and organizational skills
  • Ability to work rotating weekends and flexible hours as needed

Preferred Qualifications

  • One to two years of experience working with older adults

Schedule

Full-time position including rotating weekends. This is a non-exempt, overtime-eligible role.
