Licensed Practical Nurse, Full-Time Days, 8a-4p, Somerset/Union , Atlantic Visiting Nurse

Atlantic Visiting Nurse is seeking a Licensed Practical Nurse for a full-time position working days, 8am to 4pm, in the Somerset/Union area. In this role, you will deliver safe, compassionate home care to patients according to their individualized plans of care, working collaboratively with the interdisciplinary healthcare team and under the supervision of registered nurses.

Clinical Setting

You will provide home-based nursing care as part of Atlantic Health's comprehensive home care and hospice services. You will work within patients' homes and communities, supporting not only patients but also their families with education, expertise, and compassionate care.

Responsibilities

• Deliver safe patient care according to the plan of care as delegated by the RN
• Document all care and communication timely and within established timeframes
• Report all clinical findings and significant changes to the RN or designee, and notify the physician of significant changes
• Collaborate with the healthcare team, coordinators, and clinical managers to facilitate care planning and effective communication
• Assist the RN in patient and family communication and education
• Provide written teaching materials to support patient learning and assist in evaluating patient response to education
• Support patient safety by instructing patients and families in safety measures and following infection control practices
• Report all occurrences and seek supervision appropriately
• Maintain competencies and stay current with new techniques and standards of care
• Assist in the development of agency processes and procedures
• Provide orientation to new employees and cross-train to learn new skills
• Meet agency productivity standards
• Accept other assignments as needed to support the agency

Required Qualifications

• Current New Jersey LPN license
• Minimum of one year of experience in acute, subacute, or home care setting
• Experience using an electronic medical record (EMR)
• Excellent communication skills
• Valid driver's license

Travel Requirements

• Use of a personal vehicle is required for home visits
• Driver's license verification is required

Schedule

• Full-time, days: 8am to 4pm
