Emerald Care Center Southwest is seeking a Licensed Practical Nurse (LPN) to deliver high-quality, compassionate care to residents in our skilled nursing facility in Oklahoma City, OK. In this role, you will assess patient needs, provide direct nursing care, and collaborate with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to resident well-being.

Clinical Setting

You will work in a skilled nursing facility with potential exposure to infectious diseases and hazardous materials. Appropriate personal protective equipment (PPE) is provided.

Responsibilities

  • Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician
  • Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing
  • Assist in the development and implementation of individualized care plans tailored to meet each resident's needs
  • Maintain clear and accurate documentation of patient care and communicate effectively with residents, families, and the healthcare team
  • Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols
  • Provide emotional support and education to residents and their families, helping them understand care plans and health conditions
  • Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care

Required Qualifications

  • Valid LPN/LVN license in Oklahoma
  • Graduation from an accredited LPN/LVN program
  • Current CPR/BLS certification
  • Ability to lift and move patients, perform physical assessments, and stand for extended periods
  • Excellent verbal and written communication skills
  • Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families

Preferred Qualifications

  • Previous experience in a skilled nursing facility or similar healthcare setting
  • Additional certifications as may be required based on state regulations
