Licensed Practical Nurse

Elite Home Health, a Medicare-certified home health agency serving communities across Florida, is seeking Licensed Practical Nurses to provide skilled nursing care to patients in the home setting. As an LPN, you will deliver care under the supervision of a Registered Nurse, working within established care plans and physician orders to support patient recovery, independence, and quality of life.

Clinical Setting

You will provide care in patient homes within your assigned territory, working as part of an interdisciplinary care team. Work is performed in variable home environments with exposure to pets, smoke, infectious diseases, and weather-related travel conditions.

Responsibilities

• Provide skilled nursing care under RN supervision in accordance with physician orders and established care plans
• Monitor patient condition and report changes to the RN and care team
• Perform skilled nursing procedures within LPN scope of practice and agency policy, including medication administration, wound care, dressing changes, and catheter care
• Assist with rehabilitative and supportive care interventions as directed
• Complete timely and accurate clinical documentation in the electronic medical record system
• Document patient care, observations, and interventions performed
• Observe and report changes in patient condition, response to treatment, or concerns
• Reinforce patient education provided by the RN or care plan
• Participate in coordination with the RN and interdisciplinary care team
• Follow established care plans and notify the RN of any concerns or changes
• Participate in required training, in-services, and staff meetings
• Support patient comfort, safety, and continuity of care

Required Qualifications

• Graduate of an accredited Practical Nursing program
• Current Florida Licensed Practical Nurse (LPN) license in good standing
• Minimum of one year of nursing experience
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
• Ability to pass background screening and all compliance requirements

Preferred Qualifications

• Home health, hospice, or acute care experience

Physical Requirements

• Ability to travel to patient homes within assigned territory
• Ability to stand, walk, bend, lift, and assist patients as needed
• Ability to perform skilled nursing tasks in home environments
• Ability to use electronic medical record systems
• Ability to communicate effectively with patients, families, and care team
