The Arizona Department of Veterans' Services is hiring a Licensed Practical Nurse for the Arizona State Veterans' Home in Flagstaff. This full-time position provides direct nursing care to veterans in a skilled nursing facility setting, delivering compassionate, high-quality services within the scope of an LPN license.

Clinical Setting

You will work at the Arizona State Veterans' Home - Flagstaff, located at 2100 North Gemini Road, Flagstaff, AZ 86004. This is a long-term care facility serving veterans with varying medical, physical, and psychological care needs.

Schedule

  • 12-hour day shifts from 6:00 a.m. to 6:30 p.m.
  • Every other weekend off
  • Weekend shifts receive supplemental pay at 10% of base pay

Responsibilities

  • Administer medications and perform treatments as ordered by the physician and within scope of license, including oral, enteral, parenteral, and suppository medications, as well as sterile wound care, postural drainage, tracheostomy care, gastrostomy and jejunostomy care, and catheter care
  • Observe and report the effectiveness of medications and treatments
  • Ensure adequate supplies and medications are available on the assigned unit
  • Receive and communicate information regarding residents' medical, social, functional, and other needs
  • Participate in shift reports and communicate with residents, families, and staff to ensure appropriate interdisciplinary care
  • Complete initial admission nursing assessments and assist with Minimum Data Set (MDS) assessments, quarterly reviews, and care plan development
  • Supervise subordinate staff and assist with Activities of Daily Living (ADLs) including bathing, dressing, observations, and treatment
  • Conduct and participate in in-service training and staff development programs
  • Attend and participate in staff meetings
  • Collect and analyze data for quality improvement and infection control programs
  • Train and orient new employees
  • Perform technical duties as assigned, including specimen collection and emergency response
  • Participate in facility meetings and implementation of the Arizona Management System (AMS) to achieve continuous improvement

Required Qualifications

  • Current or temporary license to practice as a Licensed Practical Nurse in Arizona
  • Current negative TB skin test (Mantoux) within the last 12 months, or written statement from a physician, physician's assistant, or registered nurse practitioner indicating freedom from tuberculosis if previously positive
  • Ability to obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety
  • Current CPR card obtained by physical assessment

Preferred Qualifications

  • One or more years of experience in a skilled nursing or long-term care facility

Compensation and Incentives

  • Salary: $26.3083 - $31.0096 hourly (based on experience)
  • Eligible for a $5,000 hiring incentive
  • Weekend shifts receive supplemental pay at 10% of base pay
