Licensed Practical Nurse

We are seeking Licensed Practical Nurses to join our care team. This position involves providing direct patient care, conducting assessments, managing medications, performing wound care, and leading certified nursing assistants and medication technicians. You will respond to emergency situations, manage patient admissions and discharges, and ensure compliance with facility protocols and policies.

Schedule

• Full-time day shift
• Part-time evening shift
• PRN opportunities

Responsibilities

• Conduct patient assessments, follow established protocols, and report medical or behavioral changes
• Document clinical findings and maintain accurate patient records
• Perform admission duties including completing assessments, medication administration records (MAR), treatment administration records (TAR), plan of service (POS), reviewing chart setup and physician orders, and ordering medications
• Manage patient discharges by completing required documentation, notifying physicians and guardians, and arranging transportation
• Provide wound care including following protocols, irrigating, packing, and removing dressings
• Lead and supervise CNAs and CMTs while ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
