Countryside Health Care of Milford is hiring Licensed Practical Nurses for 3-11 and 11-7 shifts. In this role, you will conduct and coordinate all nursing activities on your assigned unit in a long-term care setting and active rehabilitation unit, working with a variety of patients to help them achieve optimal function or successfully return home.
Responsibilities
- Receive and give accurate, concise reports on residents and patients at change of shift
- Provide oversight and management of CNAs assigned to your unit, including daily assignments, break scheduling, and performance evaluation
- Manage all resident and patient care on the unit, including updating nursing care plans and CNA care plans with new orders and changes
- Review care plans for appropriate problems, approaches, goals, and revisions
- Review CNA documentation including vital signs, bathing, weights, intake and output, and bowel movements
- Complete nursing summaries and quarterly assessments as assigned
- Make daily rounds and maintain awareness of all resident and patient conditions, reporting pertinent data to the Unit Manager
- Notify physicians, nurse practitioners, and responsible parties of significant changes in condition
- Evaluate resident and patient needs and the effectiveness of care provided
- Promote effective restorative nursing techniques
- Maintain universal precautions and infection control policies
- Document completely and accurately using only standard abbreviations
- Administer and record medications as ordered and maintain accurate narcotic counts according to pharmacy and facility policy
- Maintain, order, and obtain adequate medications, nursing supplies, and equipment for the unit
- Perform weekly skin checks and administer treatments as ordered, documenting changes and progress
- Obtain finger stick blood sugars as ordered with accurate documentation and notification of providers if results are outside parameters
- Obtain specimens, follow up with lab, and notify providers of results
- Assist with meal trays, call lights, and alarms
- Clean, restock, and empty trash on medication and treatment carts before change of shift
- Contact providers and families as necessary
- Transcribe new orders promptly and accurately
- Admit, transfer, and discharge residents on your assigned unit and notify business office and dietary of changes
- Complete incident reports and document in the medical record according to facility policy
- Complete nursing log at end of shift
- Assist with orientation and staff development
- Participate in staff meetings and continuing education opportunities
- Deliver direct resident and patient care when necessary
- Assume supervisory responsibilities if requested by the Director of Nursing or designee
- Perform other duties as assigned
Clinical Setting
You will work in a long-term care facility with an active rehabilitation unit, caring for residents and patients with varying acuity levels and care needs.
Schedule
- 8-hour shifts
- Day, evening, or night shift options
- Full-time position
- Includes holidays and weekends
Required Qualifications
- Active Licensed Practical Nurse (LPN) license
- Ability to push, pull, move, and/or lift a minimum of 25 pounds for at least 50 feet with or without mechanical devices
- General health requirements as set forth by facility policies

Compensation and Benefits
- Salary: $29.00 to $45.00 per hour based on years of experience and shift differential
- Sign-on bonus of $3,500: $1,000 after 90 days, $1,000 after 6 months, $1,500 after 1 year
