Licensed Practical Nurse - Clinic II (Meds)

Kaiser Permanente is seeking a Licensed Practical Nurse for our Clinic II (Meds) position in Hilo. Under the direction of a Registered Nurse and medical orders of the Practitioner, you will provide direct and indirect nursing care in an outpatient clinic setting. This role emphasizes medication administration, immunization management, and comprehensive patient care coordination.

Clinical Setting

You will work in the Hilo Medical Offices Primary Care clinic, a fast-paced outpatient environment serving diverse patient populations across all age groups. The clinic operates Monday through Friday, 8:00 AM to 5:00 PM, with 32 hours per week as a regular part-time position. Travel is required 50% of the time.

Responsibilities

• Review and prepare medical records for assigned patients; identify and prepare orders for patient needs requiring practitioner authorization; accept verbal orders per policy; observe, collect, and document new patient information; inform RN or practitioners of incomplete data, new observations, or abnormal findings
• Implement basic nursing procedures per plan of care; administer treatments, immunizations, and medications as prescribed; prepare medications for practitioner use; perform point-of-care testing as authorized; provide laboratory and x-ray reports to practitioners; notify patients of normal results as directed
• Document implementation of orders and interventions using department and JCAHO standards; observe patient and family responses to care plans; report unexpected responses to RN or practitioners; collaborate to revise care plans when needed
• Perform efficiently in emergency patient care situations; follow established procedures; inform appropriate practitioners or RN
• Assign basic nursing tasks to unlicensed personnel as appropriate; provide clear directions; act as resource person; follow up to ensure care meets standards, policies, procedures, protocols, and guidelines
• Follow discharge plans developed by Practitioner or RN; process and follow up with referrals; provide instruction and pre-printed information to patients; observe and document patient and family responses
• Manage all patient vaccine data, including updating Hawaii Immunization Registry System (HIR) and Electronic Health Records (EHR) at each visit; ready immunization information prior to provider visits; store and administer immunizations according to Health Plan and VFC guidelines
• Identify patient immunization requirements per CDC age-specific guidelines; complete appropriate immunization paperwork; facilitate vaccine Health Plan and VFC inventory control; order vaccines and maintain appropriate inventories
• Utilize file maintenance to ensure newly arrived vaccines are entered in the EHR lot manager system; ensure immunization clinics comply with health plan, federal, and state guidelines; complete annual VFC enrollment training and maintain all VFC program records
• Train new and existing LPNs on emergency vaccine storage procedures; serve as liaison with the state to coordinate VFC inventory; generate reports on patients and contact them for immunization compliance and updates
• Assist in ensuring compliance with infection control policies and protocols; follow medication administration and storage procedures per Health Plan, CDC, State, and federal VFC program guidelines; provide educational materials to patients and review printed instructions
• Participate in orientation of new personnel; demonstrate sufficient knowledge, skills, and abilities in area of specialization; perform clean and sterile technique; set up for procedures; assist with clinical procedures within department scope; follow recommended processes for handling point-of-use instruments
• Maintain, order, organize, rotate, and stock supplies; schedule patients for routine care, acute care, and care gap addressing; prepare and process patient forms
• For OHS positions: Obtain certification in Spirometry, Hearing Conservation, and Substance Abuse Specimen Collection within one year of hire
• Demonstrate knowledge, skills, and abilities necessary to provide age-appropriate care and culturally sensitive service
• Perform other duties and accept responsibility as assigned within scope

Required Qualifications

• Licensed Practical Nurse (Hawaii) license required at hire
• Basic Life Support certification from American Heart Association
• Hawaii State Board of Nursing approved curriculum for LPN licensure
• Demonstrated knowledge and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication
• Ability to talk to co-workers, customers, outside vendors, and on the telephone
• Ability to read, write, speak, and understand English
• Ability to receive and give training and instructions
• Mathematical ability, attention to detail (organization, prioritization, proofing), concentration, and alertness

Preferred Qualifications

• Previous nursing experience
• Previous clinic experience
• Demonstrated knowledge and skill in word processing and spreadsheet PC applications

Compensation

Possible $5,000 sign-on bonus eligibility.
