LICENSED PRACTICAL NURSE

Citizen Advocates is seeking a Licensed Practical Nurse to deliver clinical care to individuals supported by the organization. You will utilize the nursing process to collect clinical data, plan interventions, implement care, and evaluate outcomes while maintaining professional nursing standards and interacting effectively with clients, families, and interdisciplinary team members.

Responsibilities

• Observe and assess clients, collecting and documenting current health status and medical history
• Follow physician orders including medication and injection administration
• Provide direct patient care according to organizational policies, procedures, and unit-specific competencies
• Assume accountability for patient care across various settings based on individual patient needs
• Comply with safety policies and procedures specific to your assigned system, division, or unit
• Adhere to regulations and guidelines established by the New York State Office of Mental Health (OMH), Office of Alcohol and Substance Abuse Services (OASAS), Office of Persons with Development Disabilities (OPWDD), and applicable federal and state laws

Required Qualifications

• Graduate of an accredited Licensed Practical Nurse program
• Current New York State LPN licensure
• Continuing professional education as required

Schedule and Travel

• Hours and schedules may vary based on company need and workload. Employees will receive advance notice of changes when possible.
• Travel may be required based on departmental and location needs.
