Licensed Practical Nurse

Charter Senior Living is seeking a Licensed Practical Nurse to provide clinical support and nursing care to residents in our assisted living and memory care communities in Fox Lake, IL. In this role, you will play a vital part in supporting resident health, wellness, and quality of life through direct care, clinical assessments, medication management, and coordination with the interdisciplinary care team.

Clinical Setting

You will work in assisted living and memory care environments serving senior residents. The role requires collaboration with caregivers, medication technicians, physicians, and families to ensure resident-centered care focused on dignity and quality of life.

Responsibilities

• Provide nursing care and clinical support to residents
• Assist with resident assessments and care planning processes
• Administer medications and treatments according to physician orders and company policies
• Monitor resident conditions and communicate changes appropriately to the care team
• Maintain accurate clinical documentation and medication records
• Partner with caregivers, medication technicians, physicians, and families to ensure quality resident care
• Support resident dignity, comfort, and quality of life
• Assist with compliance, infection control, and quality assurance processes
• Support and mentor caregiving staff as needed
• Foster positive relationships with residents, families, and team members

Required Qualifications

• Active LPN license in good standing
• Strong medication management and clinical assessment skills
• Excellent communication and teamwork abilities
• Ability to work in a fast-paced environment while maintaining attention to detail
• Strong organizational and documentation skills

Preferred Qualifications

• Assisted living, memory care, long-term care, or senior living experience
