Licensed Practical Nurse

Central Nursing Home is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing facility in Chicago, Illinois. In this role, you will deliver compassionate, high-quality nursing care to residents while working under the guidance of our Director of Nursing. You will play a leadership role in our care team, supporting certified nursing assistants and helping to maintain a safe, respectful, person-centered environment.

Clinical Setting

This is a skilled nursing facility providing long-term care services. You will work in a collaborative, team-oriented environment alongside nursing leadership, CNAs, and an interdisciplinary care team.

Responsibilities

• Deliver safe, high-quality nursing care to residents
• Administer medications and treatments according to physician orders and established protocols
• Monitor resident health status and recognize significant changes
• Oversee CNAs and ensure proper delegation of duties
• Create and update resident care plans with precision and empathy
• Document resident care clearly and thoroughly in accordance with facility standards
• Support a positive, respectful environment for staff and residents
• Collaborate with the Director of Nursing, Associate Director of Nursing, and interdisciplinary team members

Schedule and Shifts

Shifts available include:

• AM: 7:00 AM – 3:30 PM
• PM: 3:30 PM – 12:00 AM
• NOC (Night): 12:00 AM – 8:30 AM

Schedule includes Monday through Friday with rotating weekends. Weekend availability is required.

Required Qualifications

• Valid Licensed Practical Nurse (LPN) license in the State of Illinois
• Graduation from an accredited nursing program
• Strong clinical judgment and communication skills
• Ability to supervise, delegate, and lead a team
• Professional and reliable work performance
• Must meet standard health and background requirements
• No abuse convictions
