Licensed Practical Nurse Care Coordinator

Golden Valley Memorial Healthcare is seeking a Licensed Practical Nurse Care Coordinator to help patients successfully navigate the healthcare continuum by supporting care transitions, chronic disease management, population health initiatives, and post-discharge follow-up activities that improve outcomes and enhance the patient experience.

Clinical Setting

This position supports patients across multiple care settings, including hospital-to-home transitions and primary care environments. You will work collaboratively with providers, care teams, and departmental staff to identify, manage, and support high-risk patients.

Responsibilities

• Collaborate with providers, care teams, and departmental staff to identify, manage, and support high-risk patients across the continuum of care
• Meet with hospitalized patients to assess discharge needs, provide education, and facilitate safe transitions of care
• Conduct readmission risk assessments and follow-up evaluations for patients discharged within the previous 30 days
• Assist providers in identifying patients at moderate to high risk for readmission and implement strategies to reduce avoidable hospitalizations
• Perform post-discharge outreach calls to assess patient status, reinforce discharge instructions, identify barriers to care, and coordinate necessary follow-up services
• Maintain timely, accurate, and professional communication with patients, providers, care teams, and other internal and external stakeholders
• Manage daily worklists and assigned care coordination activities, ensuring appropriate follow-up, documentation, and communication
• Support compliance with Rural Health Clinic (RHC), Joint Commission, and other applicable regulatory and accreditation standards
• Participate in payer-sponsored quality initiatives, meetings, and educational programs as assigned
• Perform chart abstraction, quality data collection, reporting, and data submission activities to support quality improvement and performance initiatives
• Document care coordination activities accurately and timely within the electronic health record and other applicable systems
• Assist with quality improvement efforts focused on enhancing patient outcomes, closing care gaps, and improving population health measures

Required Qualifications

• Current Licensed Practical Nurse (LPN) licensure
• Strong communication and interpersonal skills
• Ability to coordinate care across multiple settings and providers
• Knowledge of chronic disease management and care coordination principles
• Commitment to patient-centered care and quality outcomes
• Strong organizational and time-management abilities

Preferred Qualifications

• Care coordination or case management experience
• Population health experience
• Ambulatory or primary care experience
• Experience working with high-risk patient populations
• Knowledge of value-based care initiatives and quality measures
• Current Basic Life Support (BLS) certification

Licenses and Certifications

• Current Licensed Practical Nurse (LPN) licensure required
• Current Basic Life Support (BLS) certification preferred

Schedule

• 40 hours per week
• Monday, Wednesday, Thursday, Friday: 6:00 AM to 4:30 PM

Compensation

• Pay range: $20.25 to $28.34 per hour
