Broward Community & Family Health Centers seeks a Staff Nurse (LPN) to work as part of the health center care team in the provision of primary health care. In this role, you will provide direct patient services including care coordination and education, perform designated independent procedures and standing orders, and support care coordination for patients within disease-specific populations in a Patient Centered Medical Home setting. You will also contribute to the development and implementation of organizational quality improvement efforts and support the achievement of PCMH recognition through data-driven patient population management.

Responsibilities

  • Provide direct patient care and services
  • Coordinate care for patients across disease-specific populations
  • Perform designated independent procedures and standing orders
  • Provide patient education
  • Participate in quality improvement initiatives and PCMH recognition efforts
  • Use data to support better management of patient populations
  • Support alignment of organizational goals with existing quality improvement efforts

Required Qualifications

  • Graduation from an accredited nursing school with a Licensed Practical Nursing degree
  • Active license to practice nursing in the State of Florida
  • Excellent communication and interpersonal skills
  • Ability to present ideas and recommendations clearly and concisely in writing and orally
  • Proven ability to problem solve and develop solutions
  • Ability to work independently and as a team member with co-workers, supervisors, and other staff
  • Flexibility in work schedule and job assignments
  • General working knowledge of utilization management, performance improvement standards and procedures, and NCQA Standards
  • Strong computer skills
  • Sensitivity in working with diverse cultural groups
