ADAPT Community Network is seeking a Licensed Practical Nurse to provide medical treatment, first aid, emergency care, and medication administration to individuals with developmental disabilities in residential settings in Brooklyn. You will work under the direction of a Registered Nurse and as part of an interdisciplinary team to coordinate and deliver comprehensive health care services.

Responsibilities

  • Provide direct health care and hygiene instruction to individuals receiving services and their families
  • Maintain accurate progress notes and health records for each client, including documentation of examinations, immunizations, illnesses, and accidents
  • Administer medications as appropriate and supervise medication storage, oversight, and ordering
  • Identify and assess communicable diseases and infections, report findings to the Registered Nurse and consulting physician, and implement protective and preventive measures
  • Provide first aid to persons in need of immediate care, with referral to the Registered Nurse or outside emergency services as necessary
  • Assist physicians in medical examinations
  • Maintain a tickler system for all appointments, schedule appointments, and follow up on physicians' recommendations
  • Attend and transport clients to medical appointments as necessary
  • Obtain consent for surgery of residents in the absence of a Registered Nurse
  • Refer all consults and diagnostic reports to the Registered Nurse for review
  • Assist the Registered Nurse in maintaining AMAP re-certification files
  • Carry a beeper and respond to on-call questions regarding physician and RN treatment plans
  • Attend scheduled team meetings
  • Stay current with changes in the nursing field

Required Qualifications

  • Graduate of an accredited program of professional nursing
  • At least one year of experience as a Licensed Practical Nurse in the delivery of community-based health services to individuals with developmental disabilities
  • Active License and current registration as a Licensed Practical Nurse in the State of New York
  • Ability to read, write, speak, and understand English
  • Good oral and written communication skills
  • Good organizational skills
  • Interpersonal skills necessary to interact effectively with coworkers, employees, the people supported, families, and outside contacts
  • Ability to maintain strict confidentiality
  • Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices
  • Willingness to work at various program sites
