The opportunity

Job Description

BoldAge PACE is seeking a Licensed Practical Nurse (LPN) to join our interdisciplinary team serving older adults in our all-inclusive program of care. In this role, you will work under the direction of the Clinic Practice Manager to administer medications, implement primary care orders, evaluate participant complaints, and provide education to participants and caregivers. You will be based in our PACE clinic and work Monday through Friday, 8:00 AM to 4:30 PM.

Clinical Setting

BoldAge PACE is an all-inclusive program of care personalized to meet the individual health and well-being needs of older adults. The program emphasizes listening to participants and their caregivers to understand their needs and desires, with the goal of helping seniors live meaningful, independent lives at home with grace and dignity.

Responsibilities

  • Perform basic nursing services within scope of practice as delegated by the Clinic Practice Manager
  • Administer prescribed medications, vaccinations, and treatments in accordance with nursing standards and document appropriately
  • Withdraw blood from participants as designated by the Clinic Practice Manager and within scope of practice
  • Observe, record, and report participant condition and reactions to drugs and treatments to physicians
  • Triage participants within scope of practice to determine priority of care
  • Educate participants and family/caregivers regarding medications and treatment instructions
  • Prepare equipment and assist physicians during treatment, examination, and testing
  • Maintain participants' medical records and fulfill PACE organization charting and reporting requirements
  • Participate in planning and executing interventions in coordination with the Interdisciplinary Team and RN Care Manager in accordance with participants' plans of care
  • Assist other clinic staff with daily appointments and emergencies to maintain timely flow of participants
  • Respond to and refer incoming telephone calls
  • Maintain exam rooms for necessary supplies and materials; ensure cleanliness
  • Monitor expiration dates of medications and clinic supplies monthly as assigned
  • Participate in interdisciplinary team meetings, clinic meetings, and other assigned meetings
  • Assist with scheduling specialist appointments and hospital admissions as assigned or requested
  • Participate in quality improvement initiatives
  • Participate in continuing education classes and required staff training meetings
  • Follow all BoldAge policies and procedures, OSHA safety guidelines, and maintain confidentiality
  • Handle potentially infectious specimens with appropriate biohazard precautions and practice universal precautions
  • Maintain a safe working environment

Required Qualifications

  • Bilingual fluency in English and Hmong
  • Graduate of an accredited school of nursing
  • Current LPN/LVN license in good standing to practice in New Jersey (New Jersey State Board of Nursing)
  • BLS certification (must be obtained within 90 days of employment)
  • Valid driver's license with minimum state-required liability auto insurance
  • Reliable transportation
  • Medical clearance for communicable diseases and all immunizations up to date before direct participant contact
  • Pass comprehensive criminal background check including federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen

Preferred Qualifications

  • One year of experience working as an LPN
  • IV certification
  • One year of experience working with a frail or elderly population (training will be provided if not present)

Schedule

  • Full-time, Monday through Friday, 8:00 AM to 4:30 PM
