Licensed Practical Nurse

Beaufort Nursing and Rehab is seeking a Licensed Practical Nurse to provide direct patient care in our skilled nursing facility. You will work 12-hour shifts caring for residents with diverse needs, administering medications, performing treatments, and collaborating with the interdisciplinary team to deliver compassionate, high-quality care.

Responsibilities

• Provide direct patient care to residents in the skilled nursing facility
• Administer medications and perform clinical treatments
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report changes in resident condition and any concerns affecting their well-being
• Document patient information, care provided, treatment, and progress accurately and timely
• Maintain current knowledge of clinical skills, policies, and procedures to ensure safe and effective care delivery
• Participate in quality improvement initiatives and contribute to continuous learning

Schedule

• 12-hour shifts with variety in scheduling to ensure continuous resident care coverage
