Licensed Practical Nurse - Baltimore, MD

Spiras Health is seeking a Licensed Practical Nurse to join our Clinical Care Team in Baltimore, MD. In this role, you will work in partnership with Nurse Practitioners to manage the overall health of a defined population of patients with complex and chronic conditions. You will provide care-at-home services, telehealth support, and two-way digital communications to help patients achieve their health goals and address barriers to care, including social determinants of health.

This position offers the opportunity to develop deep relationships with patients while delivering high-quality clinical care in their homes and through remote interactions. You will work independently and autonomously while collaborating with an interdisciplinary team of physicians, nurses, social workers, and caregivers.

Clinical Setting

You will serve patients in their homes across designated territories in Maryland, with daily travel to patients' homes based on risk levels, supplemented by telephone calls and text-based communications. Your home territory will typically be within a certain radius from your listed home address, though you may occasionally travel to other areas within the state. Standard scheduling procedures are designed to limit driving time and maximize patient care delivery.

Responsibilities

• Conduct detailed patient assessments, including vital signs and monitoring for changes in status
• Report findings and contribute to the development and implementation of care plans in collaboration with Nurse Practitioners
• Provide patient education and help patients accomplish treatment plan goals
• Identify and seek to resolve barriers to care
• Evaluate the effects of treatment plans by observing patient progress and recommending adjustments and modifications
• Communicate effectively with physicians, nurses, social workers, family members, caregivers, and other healthcare workers
• Recommend assistive equipment and outpatient or home health follow-up programs
• Conduct routine diagnostic tests as appropriate
• Maintain accurate and timely documentation of patient care and services in the EMR system per policy
• Adhere to hygiene and safety standards, including sanitizing surfaces, equipment, and hands
• Foster a supportive and compassionate environment for patients and their families
• Consistently adhere to documented policies, procedures, and standards
• Perform other duties as assigned

Required Qualifications

• Successful completion of an LPN licensing program
• Minimum 2 years of experience as a Licensed Practical Nurse
• Valid and active state nursing license in Maryland
• Excellent knowledge of nursing care methods and procedures
• Excellent knowledge of emergency care
• Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification
• Reliable and insured personal vehicle
• Reliable home internet connection
• Strong communication skills with compassion and respect
• Strong verbal and written communication abilities
• Ability to rapidly identify and gather required information to assess and care for patients
• Ability to work with patients who may have difficulty hearing, memory issues, or lower health literacy levels
• Ability to think quickly and provide information with confidence and clarity
• Ability to manage competing priorities, organize work efficiently, and meet deadlines
• Ability to work independently and autonomously
• Confident working with EMR platforms and mobile applications
• Strong technology skills
• Strong knowledge of chronic medical conditions
• Desire to collaborate with others

Preferred Qualifications

• Experience with cardiac or pulmonology specialties
• Experience with advance directives and palliative care
• Experience with clinically complex geriatric patients
• Experience with polychronic conditions and chronic care management
• Home health experience
• Bilingual proficiency (English/Spanish)

Physical Requirements

• Ability to occasionally lift office products and supplies up to 20 pounds

Compensation

• Salary: $70,000 annually
• Non-exempt position
