Avamere Riverpark of Eugene is seeking a Licensed Practical Nurse to provide basic nursing care to residents in a long-term care setting. You will work under the supervision of a Registered Nurse, administering medications, monitoring vital signs, performing wound care, and assisting residents with daily activities. This is a full-time position with day and evening shift options.

Clinical Setting

Avamere Riverpark is a long-term care facility located at 425 Alexander Loop, Eugene, OR 97401. You will work in a geriatric and rehabilitation environment, providing care to residents requiring ongoing medical support and assistance with activities of daily living.

Responsibilities

  • Measure and accurately record vital signs including temperature, pulse, blood pressure, and respirations
  • Administer medications as prescribed by physicians, following facility protocols and under RN supervision
  • Perform wound care including dressing changes and monitoring for signs of infection
  • Assist residents with personal hygiene, dressing, bathing, and mobility
  • Ensure residents receive proper nutrition and hydration, assisting with feeding as necessary
  • Record patient history and current condition
  • Observe and document changes in resident condition, reporting concerns to the RN
  • Communicate effectively with residents, families, and healthcare professionals
  • Supervise certified nursing assistants in providing basic nursing care
  • Collaborate with other healthcare professionals to ensure comprehensive resident care
  • Maintain a safe environment by following infection control and safety protocols
  • Accurately document all care provided, including medications, vital signs, and observations
  • Provide emotional support and comfort to residents and their families

Schedule

  • Full-time position
  • Day shift: 6:00 AM to 2:00 PM
  • Evening shift: 2:00 PM to 10:00 PM

Required Qualifications

  • Nursing degree from an accredited college or university
  • Current, unencumbered, active LPN license in Oregon
  • Active CPR/BLS certification, maintained throughout employment
  • Experience with electronic medical records documentation

Preferred Qualifications

  • Experience in long-term care, geriatric rehabilitation, sub-acute care, or critical care clinical settings
