Licensed Practical Nurse

This position provides direct nursing care to residents in a long-term care facility. As a Licensed Practical Nurse, you will deliver care in accordance with federal, state, and local standards while maintaining the highest quality of service. You will work under the direction of the Director of Nursing Services or Nurse Supervisor and are expected to perform additional duties as assigned.

Responsibilities

• Provide direct nursing care to residents according to established standards and regulations
• Assist residents with daily hygiene care including bathing, oral, ear, eye, facial, hair, and nail care
• Keep residents dry by changing clothing and linen as needed
• Assist residents with bowel and bladder functions and collect specimens when required
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts
• Transport residents to and from appointments, activities, and social programs
• Administer professional nursing services including catheterization, tube feedings, suction, applying and changing dressings and bandages, colostomy and drainage bag care, and blood draws
• Perform lab tests, blood pressure monitoring, and other routine assessments as required by the resident care plan
• Admit, transfer, and discharge residents as required
• Report and document any changes in resident condition or behavior to the Charge Nurse
• Assist in the care of dying residents and provide post-mortem care
• Complete medical forms, reports, evaluations, and charting in an informative and descriptive manner
• Complete and file required recordkeeping forms and charts upon admission, transfer, and discharge
• Fill out and submit accident and incident reports to the Director as required
• Report discrepancies concerning physician orders, diet changes, and charting errors to the Nurse Supervisor
• Review complaints and grievances and make appropriate reports to the Nurse Supervisor
• Receive and give nursing reports at the beginning and end of shift
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals
• Monitor assigned personnel to ensure they follow established safety regulations
• Recommend equipment and supply needs to the Nurse Supervisor
• Review care plans daily to ensure appropriate care is being rendered
• Maintain confidentiality of all resident and family information
• Treat residents, family members, visitors, and team members with dignity and respect
• Report witnessed or reported allegations of resident abuse or neglect immediately to the Administrator, Director of Nursing, or Unit Manager
• Develop and maintain good working relationships with interdepartmental personnel
• Oversee resident needs, staff, and unit operations as needed
• Comply with facility personnel, safety, and corporate policies and procedures
• Report to work on time and as scheduled

Required Qualifications

• Valid professional LPN license in good standing with the State
• Valid CPR certification
• Basic computer skills
• Ability to read, write, speak, and understand the English language
• Ability to work beyond normal working hours and on weekends and holidays when necessary
• Ability to assist in the evacuation of residents during emergency situations
• Ability to perform essential position functions with or without reasonable accommodation
• Regular worksite attendance

• Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people
• Must be able to see and hear or use prosthetics that enable these senses to function adequately
• May be required to operate an automobile and various equipment
