Licensed Practical Nurse

Tennessee Quality Care is seeking a Licensed Practical Nurse for private duty home health care. In this role, you will provide personalized medical care to patients recovering from surgery or illness, managing difficult health challenges, or receiving end-of-life care. You will work independently in patients' homes, implementing care plans and delivering compassionate, high-quality nursing services.

Responsibilities

• Provide direct patient care as defined by the state Nurse Practice Act
• Implement the plan of care initiated by the registered nurse
• Assess patients and provide education to patients and families regarding self-care techniques and information pertinent to diagnosis and plan of care
• Provide accurate and timely documentation consistent with the plan of care
• Provide health care instructions to patients as appropriate per assessment and plan of care
• Coordinate home health services and report identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse or Clinical Supervisor
• Use and prepare equipment and supplies effectively and efficiently while adhering to aseptic technique
• Participate in personal and professional growth and development
• Perform other duties as assigned by the registered nurse

Required Qualifications

• Graduate of an accredited practical nurse or vocational nursing program
• Three years of nursing experience
• Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in Tennessee
• Valid CPR certification
• Reliable transportation, current driver's license, and appropriate automobile insurance

Preferred Qualifications

• Community/home health experience
• Medical/surgical nursing experience
