Licensed Practical Nurse (7p-7a)

Award Care Health Group is seeking Licensed Practical Nurses (LPNs) to provide direct patient care in a skilled nursing and rehabilitation facility. In this role, you will assist residents and patients with activities of daily living, implement individualized treatment plans, and supervise certified nursing assistants (CNAs).

Clinical Setting

You will work in a health and rehabilitation center (skilled nursing home) providing nursing services to residents throughout their course of treatment.

Responsibilities

• Provide direct patient care to residents and patients
• Assist in the implementation of individualized treatment plans for each assigned resident
• Supervise CNAs and participate in their evaluation
• Oversee the Falls Program and Pressure Sore Program
• Conduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment
• Perform other duties as assigned

Schedule

• 12-hour shifts: 7p-7a
• PRN shifts available

Required Qualifications

• Current, unencumbered, active state LPN license

Preferred Qualifications

• Experience in skilled nursing or rehabilitation facilities
