Licensed Practical Nurse 6a-6p (Mon-Thurs)

Onyx Care of Farmerville, a post-acute rehabilitation center in Farmerville, LA, is seeking a Licensed Practical Nurse to provide patient-centered care on a day shift schedule. This is a full-time position working Monday through Thursday, 6:00 AM to 6:00 PM.

Clinical Setting

You will work in a state-of-the-art post-acute rehabilitation facility providing skilled nursing care to residents with a focus on individualized attention and innovative technology.

Responsibilities

• Provide high-quality, patient-centered care to residents, promoting their physical, emotional, and psychological well-being
• Administer medications, treatments, and therapies as prescribed by the medical team
• Monitor and report patient progress, identifying areas for improvement and developing personalized care plans
• Collaborate with the interdisciplinary team to ensure seamless care transitions and effective communication
• Maintain accurate and detailed records of patient care, medication administration, and treatment outcomes
• Perform routine nursing tasks, including vital sign monitoring, wound care, and assistance with activities of daily living (ADLs)

Required Qualifications

• Current LPN/LVN license in the state of Louisiana
• High school diploma or equivalent
• Minimum 0 years of experience in a post-acute rehabilitation setting or similar healthcare environment

Preferred Qualifications

• Associate's degree or higher in nursing or related field

Schedule

• Monday through Thursday, 6:00 AM to 6:00 PM
