Education

  • Diploma from an accredited School of Nursing

Licenses

  • Active Licensed Practical Nurse (LPN) license in good standing from the state of Oklahoma

This position provides direct nursing care to residents in a long-term care and skilled nursing facility setting. You will supervise the day-to-day nursing activities performed by CNAs and CMAs while maintaining compliance with the Oklahoma Nurse Practice Act and all federal, state, and local regulations.

Responsibilities

  • Provide direct nursing care to residents and supervise nursing staff activities
  • Coordinate nursing care and resident services to ensure comprehensive care delivery
  • Prepare and administer medications following the 5 Rights of Medication Administration
  • Take and record vital signs and perform personal care tasks as needed
  • Administer professional services including catheterization, tube feedings, suctioning, wound care, colostomy and drainage bag management, blood draws, range of motion exercises, and end-of-life care
  • Develop, maintain, and implement care plans; conduct routine checks to ensure CNAs and CMAs follow physician-outlined plans
  • Receive and transcribe physician orders to resident medical records
  • Chart all communications with physicians, medical professionals, residents, and families
  • Record new or changed diet orders and communicate with dietary department
  • Ensure complete medical charting and documentation upon admission, transfer, and discharge
  • Complete accident and incident reports and notify physicians of changes in resident condition
  • Assist with discharge planning and conduct periodic reviews of discharge care plans
  • Participate in quality assurance programs and government surveys/inspections
  • Maintain infection control protocols and use appropriate personal protective equipment
  • Respond promptly to resident call lights and maintain high-quality resident care and comfort
  • Communicate effectively with residents, families, staff, and management
  • Maintain confidentiality of resident records and information
  • Protect residents from neglect, abuse, and mistreatment; report incidents to the Director of Nursing immediately

Schedule

  • Monday through Friday, 3:00 PM to 11:00 PM shift
  • Shift differential pay included
  • Full-time position

Required Qualifications

  • Active Licensed Practical Nurse (LPN) license in good standing from the state of Oklahoma
  • Minimum 1 year of experience in a skilled nursing facility or long-term care environment
  • Diploma from an accredited School of Nursing
  • At least 18 years of age
  • Ability to pass a background check
  • Ability to read, write, and follow oral and written directions at a level necessary to accomplish the job
  • Ability to relate positively and favorably to residents and families
  • Ability to work cooperatively with other staff members
  • Capability to maintain regular attendance
  • Excellent communication and teaching skills in both individual and group settings

Physical and Mental Requirements

  • Genuine interest and compassion for caring for elderly and disabled individuals
  • Ability to lift, bend, climb, reach, push, and pull as required by the position
