Licensed Practical Nurse 1199

Woodbury Heights Nursing and Rehabilitation Center is seeking a Licensed Practical Nurse to provide compassionate, patient-centered care to residents in our long-term care setting. You will work as part of a dedicated nursing team, collaborating with RNs, physicians, and other healthcare professionals to ensure the highest standard of resident care.

Responsibilities

• Provide compassionate, patient-centered care to residents, helping them live comfortably and with dignity
• Administer medications and treatments as prescribed by physicians
• Monitor and document vital signs, symptoms, and changes in resident conditions
• Assist in developing and implementing personalized care plans
• Collaborate with RNs, physicians, and other healthcare professionals to ensure the highest standard of care
• Respond to medical emergencies and provide immediate support
• Maintain accurate and up-to-date medical records
• Promote a positive, supportive environment for both residents and team members

Required Qualifications

• Valid LPN license in New York State
• Current CPR and First Aid certification
• One year of experience in long-term care

Schedule

• Must be able to work every other weekend
• Available shifts: 7am to 3pm, 3pm to 11pm, or 11pm to 7am
• Weekday shifts also available
