License Practical Nurse - Home Health

Signature Healthcare at Home is seeking a Licensed Practical Nurse to provide skilled home health nursing services on South Whidbey Island, including the communities of Greenbank, Clinton, Columbia Beach, and Coupeville. This full-time position offers Monday through Friday day shift work with self-directed scheduling based on patient needs and territory. Responsibilities

• Provide direct patient care according to the plan of care, including wound care, catheterization, disease process education, and medication management and education
• Work with the Registered Nurse Case Manager to develop the plan of care in partnership with the patient's primary care provider
• Complete timely documentation, evaluate patient needs, and record pertinent information into the chart
• Work outside the primary assigned area as needed due to staffing and census fluctuations
• Participate in on-call duties, typically 3-4 weekday shifts per month and 1-2 weekend days per month

Required Qualifications

• Graduate of an accredited school of nursing
• One to two years of recent acute care experience in an institutional setting
• One to two years of recent home health experience
• Currently licensed as a Practical Nurse (LPN) in Washington State
• Valid CPR certification
• Valid driver's license with maintained automobile insurance
• Access to an automobile for agency-related travel, including visits to community partners and patients

Preferred Qualifications

• Experience in home health or post-acute care
• Familiarity with Homecare Homebase EMR

Schedule and Compensation

• Full-time position, 32+ hours per week, Monday through Friday
• Hourly rate: $37-$42 depending on experience
• Mileage reimbursement provided
