Aviata Health Group is seeking an Infectious Disease Nurse Practitioner to serve as the primary clinical provider in skilled nursing and long-term care facilities. In this in-house role, you will lead infectious disease programs, participate in residents' plans of care, and build collaborative relationships with residents, families, facility staff, and providers while serving as an integral member of the clinical leadership team.
Clinical Setting
You will work in skilled nursing facilities and long-term care environments, focusing on proactive healthcare delivery and prevention of critical health issues and re-hospitalizations in the geriatric population.
Responsibilities
- Perform comprehensive physical examinations and evaluate changes in patient conditions
- Prescribe medications and treatments as needed
- Develop patient care policies and facilitate referrals to specialists
- Coordinate advanced care planning and discharge visits
- Ensure timely communication and documentation of patient assessments, treatment plans, and follow-ups
- Support the physician-patient relationship and address gaps in care
- Meet a minimum of 12 patient visits per day, with bonus opportunities for exceeding this target
- Work collaboratively with the interdisciplinary team and physicians to ensure high-quality service delivery
Required Qualifications
- Active, unencumbered Florida Nurse Practitioner license
- Master's degree in nursing from an accredited program
- Strong clinical background in infectious disease
- Ability to engage and communicate effectively with elderly and chronically ill patients and their families
Preferred Qualifications
- Geriatric or adult medicine experience
- Experience in skilled nursing, long-term care, hospice, or palliative care settings
- Knowledge of Medicare, Medicaid, and health plan benefit structures
Schedule
- Flexible full-time schedules
