The opportunity

Job Description

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations across a defined coverage area in Idaho. In this role, you will visit patients in their homes to conduct comprehensive health assessments, engage in one-on-one consultations, and help bridge gaps in care. This is a travel-intensive position requiring up to 100% travel time, which may include overnight hotel stays, car rentals, and flights depending on the coverage area.

Clinical Setting

You will work primarily in patients' private residences, meeting health plan members where they are. These environments are diverse and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied home settings.

Responsibilities

  • Conduct in-home health evaluations lasting approximately 45 minutes per visit
  • Review medication and medical history with each patient
  • Perform physical evaluations
  • Administer diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, and bone density screening
  • Use an iPad and intuitive clinical workflow to document evaluations
  • Provide education to help patients better understand their health and take better care of themselves
  • Answer patient health questions and concerns
  • Conduct virtual visits as needed
  • Work flexibly to travel locally and within licensed states
  • Build trusted relationships to improve health outcomes

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

  • Active, unrestricted license(s) in coverage area(s)
  • Board certification
  • Ability to obtain and maintain multi-state licenses in coverage area as required
  • Ability to cover all counties within state and flex to other areas of business need

  • Full-time position
  • 40 anticipated weekly hours
  • Travel requirements: Up to 100% travel time across defined coverage area, with potential overnight stays
