In-Home Nurse Practitioner or Physician Assistant (Per Diem) - San Francisco, CA

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations. You will visit patients in their homes to conduct comprehensive health assessments, engage in medication and medical history reviews, perform physical evaluations, and administer diagnostic tests as ordered by the patient's health plan, such as diabetic eye exams and spirometry.

Clinical Setting

Your primary workplace is the private residence of health plan members. You will meet patients where they are, in diverse home environments. You must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic and preventive services as ordered, including spirometry, diabetic eye exams, and bone density screening
• Use an iPad and intuitive clinical workflow for each evaluation
• Provide patient education and answer health questions
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed
• Work collaboratively with Signify Health clinical colleagues

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

Compensation

• Hourly base rate: $46.03 - $99.14
• Additional incentive structure based on visits completed and other incentives as applicable
• Actual base salary offer depends on experience, education, geography, and other relevant factors
