In-Home Nurse Practitioner or Physician Assistant (Per Diem) - Maui, HI

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Maui, Hawaii. In this role, you will visit patients in their homes to conduct comprehensive health assessments, helping bridge gaps in care and connect individuals with appropriate resources for their unique health needs.

Clinical Setting

You will work primarily in patients' private residences, meeting health plan members where they live. As an in-home clinician, you will encounter diverse home environments that may include household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic and preventive services as ordered by the patient's health plan, including spirometry, diabetic eye exams, and bone density screening
• Use an iPad and intuitive clinical workflow to document evaluations
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed
• Build trusted relationships with patients to support better health outcomes

You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

Schedule

• Part-time, per diem position
• Anticipated weekly hours: 12

Compensation and Benefits

• Hourly base rate: $50.22 - $108.15
